Parents/Guardians due to current school closure to better assist you during this time please see the following regarding Student Registration and Withdrawal until further notice from the Dekalb County School District.
 
New Registration: For students that have never attended a Dekalb County School before or did not attend for the 2019-2020 school year. Click the blue registration button to the left and follow the instructions to complete the Online Registration Application.
 
Transferring from another Dekalb County School: Students transferring from another DCSD school MUST Complete the Household and Student Registration Packet and email it to [email protected].  Email completed packets with your lease/mortgage document AND current utility bill (electric, gas, or water) and your photo ID to [email protected]
 
To Update Current Student's Information: Current Student/Annual Verification-To complete the annual verification process Parent/Guardian must have an Infinite Campus Parent Portal account to verify/update their student(s) information. Please visit the DCSD website: here
 
To Withdraw: If you wish to withdraw your student, complete the withdrawal form (PDF linked below), sign and submit with Government Issued Photo ID via email to [email protected]

Immunization Information:
IMMUNIZATION PAPERWORK MUST BE SENT TO THE SCHOOL [email protected]
By state law, a valid certificate of immunization (Form #3231) or a notarized statement of religious exemption (Form #2208) is required for enrollment. The certificate of immunization must be completed by the health department or your private physician. A valid form #3231 must be marked with either "Date of Expiration" or as "Complete for School Attendance." (A certificate marked with a "Date of Expiration" expires on the date indicated. A current certificate must be submitted within 30 days of expiration.) A medical exemption, if applicable, should be noted on form #3231 with a current date of expiration. You may request a form from your school for a religious exemption.

Please review the immunization requirements for students entering 7th grade and a TDAP-Meningitis parent fact sheet. The new vaccine requirements apply to students who are born on or after 1/1/2002 AND entering 7th grade. The requirements also apply to students who are new entrants to 8th-12th grade. New entrants are students who have never attended any school in Georgia or have not attended any school in Georgia for at least one full school year.
 
Students also must have evidence of vision, hearing, and dental, and nutrition screening obtained in the past year. This information should be on Georgia Form #3300 (rev. 2013), available from the health department or your physician and dentist and dated within the last 12 months. Letters from appropriate healthcare professionals and out-of-state certificates are acceptable if completed within the last 12 months and stapled to the state form. 
Visit one of the health department offices in DeKalb County to obtain required screenings and immunizations for a fee.
All students entering 6th grade are required to meet the following:
1. Two (2) doses of Measles vaccine, two (2) doses of Mumps vaccine, and one (1) dose of Rubella vaccine or laboratory proof of immunity against Measles, Mumps, or Rubella.
2. Two (2) doses of Varicella (chickenpox) vaccine or documentation or laboratory proof of immunity. All students entering 7th grade are required to meet the following:1. One (1) dose of Tdap (tetanus, diphtheria, pertussis)
All students entering 7th grade are required to meet the following:
1. One (1) dose of Tdap (tetanus, diphtheria, pertussis)
2. One (1) dose of meningococcal conjugate vaccine.? Contact your health care provider or local health center to receive any needed vaccinations and to obtain a signed Georgia Certificate of Immunization (Form 3231) with "Complete for School Attendance" marked.
